Dentist says DO NOT CROWN YOUR TEETH! – Proves it with a Clinical example!

– What’s up guys? This is Dr. O from Innovative Dental, and pleasure to have you here today. Man, I hope you’re smiling,
keep smiling right? Anybody like that cheesy
line I give at the end? No, stupid? Okay, well, I’ll have to rethink it because I liked it. Dang it. Hope you’re tuned in and
gonna listen a little bit, learn about dentistry, because I’m super passionate about it. Hopefully you get that from the video. We’re gonna talk about, why you shouldn’t crown your front teeth, and I’m gonna give you a clinical example. My first video I created
was Do Not Crown Your Teeth. And I explained why front
teeth hardly ever need crowns. There’s a lot of questions from that. Even some dentists have some questions of why that’s my thought, and questioning my reasoning
behind creating the video. And I wanna help bring
some clarity to that so, I’m gonna use a clinical example, and then after that I’m gonna give you maybe three or four reasons why a crown may be necessary
on your front teeth, so you can maybe go okay, those are really the three or four that I might need a crown. First, let’s look at
this clinical example. I want to use this one because it’s pretty impressive, right? You look at this, and
you look at that photo and you go, man, that looks
like extractions, right? Can we even save those teeth? Probably gonna need some root canals, definitely gonna need some crowns, maybe even extractions
if they’re even saveable. And if you look what we did here, we went ahead and, first
let’s remove all the decay. So in this photo you’ll
see decay is removed. We wanna just see what
the tooth structure’s left after decay is gone. And you’ll notice, we didn’t
get into any of the nerves. So, all of the nerves
were healthy and vital. No root canals were needed here. So, pretty cool, right? Saved the guy let’s say, four root canals, 4,000 dollars roughly in our area. Next you’ll see that we restored
the teeth with composite. So we reshaped the tooth structures so that a way of veneer could be created and bonded to the tooth. And so you’ll see, we reshape it. By the way, the blue dye that you see is purely to show any amount of decay that’s left on the tooth. And we use that to make
certain we get everything. We don’t wanna leave any decay behind. One of the cheesy sayings I love is, no bugs left behind. That’s kinda dumb, no
bugs left, ah, you get it? Bacteria, bugs? No bugs left, it’s stupid, alright. Well anyways, so, we definitely didn’t
wanna leave any decay. Then we build it back up, and then we image with
our machine and create, and you can see some of the
machines behind me here. Those are CEREC machines, those allow us to image in our office and create veneers right in
our practice, single visit. So this smile was completed
in under four hours, without any root canals, all veneers. And matter of fact, if you
would have crowned these teeth, there’s no way you could have saved them. And the reason you
couldn’t have saved them is because you would have cut
all the enamel off the back, and you would have had hardly
any tooth structure left after crowning these teeth. Many dentists would maybe want to take and do root canals electively, and put pins inside the tooth. We chose not to do that
because I feel like teeth are healthier when they don’t have these pins inside of them. I feel like they’re
stronger, gonna last longer. By the way, this treatment, I know there might be some that go, well how long does that last? Did you do that last week? This guy has been, I fixed
this smile five years ago. So this is a smile five years
ago, still smiling today. Comes in, awesome guy. Here’s the before of his smile
and kind of how he looked, and his confidence, and
then look at him now. And yeah, that chubby
guy next to him is me. But that was five years ago,
after we fixed his smile. Pretty fun to share that story. Hopefully that clinical
example helps you go ah-ha, I understand what you mean when you say, teeth don’t really need
crowns, in the front at least. There’s a lot of benefit to a veneer, we save all that enamel. And if you want more on that, definitely go to the video link here, I’ll pry put it right here. Link to that, and watch the video of how I explain why crowns are probably not necessary for you on your front teeth. Now let’s get to the four reasons why you may wanna crown your front teeth. So these are the exceptions to that rule. The first one would be, an existing crown. I started realizing that
man, hardly any teeth, I don’t crown hardly anything unless it’s an existing crown I’m repairing. So, you know, existing crowns,
you have to crown them. The damage has already been done. The amount of tooth structure
has already been removed, or the decay is already so much that the crown was
necessary to begin with. And so a crown is necessary to replace it. Can’t go backwards, right? You can’t go to a crown, and
then go back to a veneer. So, once it’s crowned it
needs to be crowned again. The other one would be a root canal, if you had a root canal, and
it’s quite a bit of decay, and it needs a crown or
something to cover up the decay, I might wrap around and cover the access. On the backside of the tooth, there might be a little hole where the root canal was performed. I would cover that with the crown. I would not grind all the way
down to the gum line though, like a traditional crown which is really just more
tooth removal than necessary. So, that’s how I would
treat a root canal tooth. The other one would be severe decay, but man, it’s hard to have decay that actually goes around
the backside of the tooth. It just doesn’t happen very often. Every once in a while you’ll see it if somebody has like an enamel defect or a groove in the backside of the tooth, and it’s
pretty significant decay. At that point, definitely, a
crown would be a good option for that patient in that scenario. The other one I had somebody ask me is, I had some questions like
a chipped tooth, right? What do you do with a chipped tooth? I can’t see any reason why a chipped tooth should ever need a crown. It’s a really common mistake maybe I would say in diagnosing. Chipped teeth, veneers,
or just resin fillings. If the fillings keep failing, which in this gentleman’s story, that’s what was happening with him. I recommended, I said hey, you oughta encourage the dentist to maybe be able to do a veneer
for you rather than a crown if you just keep having a
filling that’s chipping. That way you save more
of your tooth structure. The other example would be, severe wear. Now, if you have severe
wear on your teeth, have kept wearing each
other down, like this, then what’s happening is over time, you’re filing down the
backside of your top teeth. If you go and crown those teeth,
you’re gonna have to remove the amount of porcelain thickness from the backside of your top teeth. So, crowning initially, would be a mistake and not a good recommendation
for tooth wear. So you don’t wanna crown initially, what you wanna do is you wanna
use some sort of technique, whether it’s Invisalign or
braces to create separation. So create space. So we wanna take what’s no space, and what’s ground away in your tooth, create some space, now we can rebuild the tooth structure there. And I can make a video
that maybe helps better clarify that but, don’t go in with tons of tooth wear and expect to get crowns unless
they’re gonna open the bite and change the vertical dimension. Otherwise you’re getting too much grinding into a tooth that’s already worn. What that is likely to
do is actually lead to a root canal or tooth pain from the fact that you just
drilled so far into the tooth that was already worn
away by your lower teeth. So, tooth wear, definitely
gonna need a crown maybe down the road, but create
the separation first with maybe like Invisalign or braces, then choose to crown the tooth. Alright, so, hopefully that helps with a clinical example of why you should not
crown your front teeth, and showing you that hey look, we can take a smile that
almost looks like dentures, and we can restore it with technology that is readily available, and techniques that don’t
require so much removal of the tooth structure. And then I gave you the four reasons why you wanna maybe consider
a crown if it’s necessary. One being that you have a existing crown, the other one being that
there might be a root canal, the third being that there’s severe decay the covers the backside of the surface, and then the fourth one
would be severe wear after you get some sort of alignment or separation created from
braces, maybe Invisalign, before you would then
have those teeth crowned so they’re fixed and not
have to be ground into, like they would if you didn’t
get that separation done. So, I hope that helps
make sense of a video that seems to get a lot of attention. And hopefully it’s helping
people understand stuff. Obviously the whole point of the channel is to take, what I fell has been not a lot of innovation in the way that we’ve
done things in dentistry. And helping to think through
why we do what we do, and provide the latest for our patients. And so, one of those would be rethinking and retooling our brain to go, we should be conservative, and let’s save as much
tooth structure as possible. And let’s make things last
longer for our patients. Rather than being so
aggressive with the hand piece. So, hopefully you’ll like

  • "I feel like pins weaken teeth" – Please practice evidence based dentistry. In the right application, posts serve a very important function. You did a great job with this case, but don't discount the need for crowns. This patient's OH was horrendous and it sounds like he's done a fantastic job turning things around. Unfortunately, most patients with poor OH aren't willing to make that change

  • You’re not doing anything differently you’re veneers are more close to 3/4 crowns. Crowns have a place as well as true veneers and your 3/4 restorations. All of these options cost around the same so luckily cost isn’t a major factor right????

  • I unfortunately am one of the rare tooth cases. I have decay on my far back top right molar that wraps around my tooth :/ I hope it won’t hurt.

  • My doctor keeps telling me I need crowns in my front teeth. I have always said no, because it never made sense to me, and by watching this, I feel better. I just wish I could afford invisaleigns, and afterward get veneers to solve my dental crossbite issues.

  • This may sound unreasonable because it "seems" over the top anxiety over a"small thing"but I can honestly state that the crown on my front upper tooth has cause me more collective grief than practically all my other health problems which trust me ARE significant (multiple compound fractures , cirrhosis of the liver/hepatitis/ skin grafts/collapsed (punctured via ALL ribs broken on one side) …3rd degree Burns and more ….so you say "seriously? HOW can a well done fricken CROWN cause you so much grief???" Well I'll tell you ….it is because it bugs the CRAP out of me EVERY damn day since 1979….40 years of all day "I HATE it!" ..I can FEEL it and unlike all my other crowns it has this annoying sensation ….the dentist looked at me like I'm nuts and finally agreed to replace it with another porcelain crown (first was gold or whatever) It was a big improvement being sleeker in the back but after a while it also bugs the crap out of me ….I don't know why I just don't have a root canal….which of course dentist strongly did not like …I just HATE IT!…..Now YMMV but if I was you if at all possible? NO crown on the top front teeth ….DO NOT DO IT …dont let them sell you on it!

  • Where are you located. Springfield Missouri? I need a trustworthy dentist that won’t kill me (literally) ? I have to be put out in order for you to work on my teeth, I almost died in the dentist chair and I’m scared to death to go. I have many caps that have decay under the root canal and would like to be able to go and get them fixed if possible. I will cry and shake just to have x-rays. Novocain does NOT work on me. I still have the drill bit that broke off while a dentist was doing a root canal in one of my molars. No gas either. I do take a medicine to help calm me down so I can go to the dentist. Put I need to have trust in the doctor and understanding about this. I’m not just saying I’m a coward. I am one, due to a dentist who wouldn’t listen to me when I told him I have long roots, and. Novocain don’t work on me. He just about killed me. I passed out and he kept on working to the point I was in convulsions. Then he broke the drill bit and got mad at me while I was passed out. I woke up to his anger, and tooth dust and smoke all around me. You can go from there.

  • What about a dead molar (the one most front upper jaw?)
    The dentist said it would be better to crown it because it would hold up better but looking on the Internet it seem like opinions on that may be divided.

  • can i ask where is your evidence that with a proper diet these teeth could not have regrown back to normal naturally? where is a study disproving that? and how can you say there is no conflict of interest if your income derives from diseased,decayed teeth?

  • My dentist suggest to do crowns on my front teeth, she said because I have too many fillings for years of chipping. She also said that veneers will brake with my issues on biting when sleeping. What do you think about that, crowns or veneers??? Thanks!

    Just imagine after a SINGLE visit to Dr. Hessam for an initial visit for exam, X-rays, and light teeth cleaning…then his seemingly UN-trained dental hygenist BROKE OFF a BIG 20-year old porcelain filling from your molar…(see dental X ray) and OVERNIGHT, you SPIT OUT what you thought was loose chicken bones for din din at home…KA-THUNK!, ka-THUNK!…KLUNK!….weird sounds…of white gravels bouncing OFF the metal sink wall surface…THREE different times…in ONE night…

    What is WORSE is the Dr. HESSAM damages the normal intact teeth with help of this nervous, UNTRAINED assistant Jane…then…STICKS YOU with a $3000 bill for non-existent crown work.

    Can you say, "SCAM?"

    But NO…those were solid CHUNKS of white chipped OFF  fillings flying OUT of your mouth bouncing OFF the metal sink!! You freak OUT…Then, you felt with your tongue…a NEW Grand Canyon in your molar…a GIANT HOLE that was NOT there before!…WTH??

    So…you email and CALL Dr. Hessam several times, leaving VM messages and mentioned INTENSE dental pains radiating OUT throughout your gums and skull…WORSE pain EVER!…But…NO reply…Dr. Hessam just ignores you..never replies back….just as he CONDONED the gross negligence of his dental assistants…

    Instead, in ONE week…Dr. HESSAM  decides to suddenly FIRE you as client and fully refunds all your dental fees…and mails it with a Dr. John letter, saying he is dismissing you as his client FOR NO sound reason….the whole time you are trying to inform him of what happened…

    What was WORSE…was when his seemigly UN-trained and RECKLESS hygienist who TALKS TOO MUCH was distracted and DRILLED into your molar with POWER TOOL, INSTEAD of cleaning it…she DAMAGED that tooth FOREVER…you watch as she runs over hastily to the supervisor in another room and they INTENTIONALLY CUT OFF the power to the dental equipment or pretend to….she looked like a scared deer in the HEADLIGHTS..too LATE…ain't it??

    …Jane, was her name…she runs back into the room and REFUSES to finish the dental cleaning…tells you they "suddenly lost power" YET all of the office LIGHTS were still ON…and that you would have to LEAVE the office IMMEDIATELY!…say WHAT???  Dental assistants NOT CERTIFIED??. Dangerously INCOMPETENT. Doctors office should ONLY HIRE certified assistants Not INCOMPETENT dental hygienists,  right?

    Just minutes BEFORE during the X-rays…all of your teeth were ALL INTACT with NO HOLE…but when you go home…you get a HUGE HOLE and HELLISH dental pain in YOUR SKULL!…Tooth must NOW be YANKED OUT…NOT fixable…it is NOW a split into TWO cracked tooth…NOT to mention ONGOING toothaches and chewing PROBLEMS…OwwwWWWW…you KNOW the kind of PAIN that makes you CRY…

    Just how SHADY can these folks be?…..Covering up gross NEGLIGENCE is just NOT ethical NOR right…come ON Dr. HESSAM…man Up and take responsibility…you KNOW your assistant SCREWED Up ROYALLY…but NO…took EASY way OUT…you know cover up = NO BUENO senor…

  • I went to a Dentist and he said I needed 3 crowns and a root canal, but they want to drill around my tooth and make it smaller and weaker, then crown it. Thanks for giving us alternatives to that stupid procedure

  • I have a front tooth that is a crown. It was broken in half. The tooth next to it has multiple horizontal cracks in it. It’s never broken but you can see the cracks when I shine a light in my mouth. I want veneers so by what you are saying, my crown will need recrowned and the one next to it will need crowned for structure issues. The rest of my teeth should only need veneers?

  • I wish i knew this several years ago. 60k later, nothing but extractions, posts and root canals due to a genetic condition. All the work failed. All. And my entire 20s of everyone thinking i was on meth crack. Still in debt to this day with the worst case scenario happening. The hardest wasnt the money or the pain in the 60 plus procedures. It was the look on how ppl judged once the crowns fell out and everything started failing.

  • But veneers are not covered under insurance and root canals or crowns are; if I could get veneers under insurance I totally would. My dentist wants to crown my front 4 teeth and I wish wish wish that insurance would cover a veneer but alas crown is my only option unless I can find like $8000 to pay out of packet for the veneers- which I cant.

  • My Dentist just did two back crowns. Upper Right 2nd & 3rd from the back tooth. Both Crowns are Wiggly semi loose. I went to him and he tells me Crowns Always will have some play in them. I have never know anyone that would say this. I have had my front tooth upper left Center tooth crown since I was in sixth grade. It had a Root Canal. My Cap was never loose ever. It fell off maybe 7 years ago and I had the another dentist put it back on. AT 52 this tooth is loose now. Thats when its time that it will come off again. I am not stupid but my dentist is such a nice guy. Funny thing, I went to pay and the two ladies were talking and the one said to the billing lady which one are we doing and I heard the billing lady say we will do the Bridge later. This is just the crowns. So there are two crowns. side by side they are both lose since he did the JOB in 2017 this is now NOV 2019. So I think they are possibly cheating people. I went in for one tooth and got the 1st one fixed. Well Later on I was cleaning my teeth with my PICK and found a Hole near the GUMS on the Side next to the CROWN. I was like ok. Either he hit that tooth on purpose or by mistake. I know every inch of my mouth and I am always looking in my MIRRORS and using my PIC to see my TEETH..

  • Where you located doc I stay in Richmond California and I wanted to save my tooth but had a cavity and I told the doctor to save it and clean the cavity go through it fill it up cuz I know I wasn't too close to the nerve but he told me some BS about if we go stop route later on you'll have pain insensitive teeth and it's better if you get a crown so now I have a whole he took all the nerves didn't finished I got to go back in two or three weeks to for crown and yeah I really wanted to save it I would like to see if I can fax you the pictures and if you can give me an advice cuz I got two more crowns probably three to do and I want to save them

  • I am PISSED my dentist did a crown for a chipped tooth. I wish you were in NYC, to see if I was viable for a veneer. I still have the nerves in my tooth so it wasn't filled down to much.

  • I would love to know how this man did not need any root canals…. I got braces at 12 and they ruined my teeth, it took me 12 years to remove them.. Now I have 1 pulled tooth 2 others need to be pulled 2 root canals and many fillings need to be done.. I’m in extreme pain everyday!!!!! I’m only 26🤦🏾‍♀️… My mouth has no chance..
